![]() The New York City Department for the Aging (DFTA) funds two Selfhelp case management programs, Queens North and Project Pilot. These programs are only funded to serve specific areas and seniors, who live in these locations, are entitled to come in and be evaluated to determine what services they may need to continue living independently within their community. Client applications are then processed so that DFTA-funded services, such as home care and Meals-on-Wheels, are available to those who qualify. Selfhelp Kensington serves seniors living in Brooklyn's zip code 11218, and Selfhelp Project Pilot serves seniors living on Manhattan's upper west side. DFTA funds many such programs throughout New York. Selfhelp manages two of them; Selfhelp Kensington Program and Selfhelp Project Pilot. |
