Selfhelp is a leading provider of home care services in the New York metropolitan area, Nassau County and Suffolk County. Selfhelp Community Services operates two types of home programs that you can directly access and four other contracted services through other agencies. When New Yorkers need care at home, they call Selfhelp.

To access care directly, call 1-888-781-4300 and provide:

a. Patient’s name, address, social security number,and
    telephone
b. Physician’s name, address, and telephone
c. Insurance information (CHHA only)
d. Primary and secondary diagnoses
e. Specific services needed

Direct Services

Certified home care for individuals with or families affected by HIV and AIDS or current Selfhelp client*.
Community licensed home care services for the frail elderly, or chronically ill
Medicare, Medicaid, ADAP or other insurance companies pay for services
Consumers pay for services
