Selfhelp Senior Source is an innovative private care management program that plans, accesses, coordinates and monitors a wide range of services that meet the needs of older adults and their families. Beginning with a comprehensive assessment of health, psychosocial, environmental and social service needs, Senior Source professional care managers work with the client, and family or caregiver to research and organize an individualized package of services and resources that will enable the client to remain independent. The care manager is usually an experienced geriatric social worker or nurse who focuses on the needs of elders, and can be retained by the client, family members, care givers, or guardians. He/she communicates with clients, caregivers and families regularly. Selfhelp also offers a unique pre-planning feature for clients who don’t have an immediate need, but wish to have a plan in place for the future.
