• Please make checks payable to Selfhelp Community Services, Inc.
• If there is a specific program you wish to support, please include the information with your check.

CREDIT CARD

• Your full name as it appears on your credit card.
• Your credit card provider. We gladly accept Visa, MasterCard, American Express and Discover.
• Your credit card expiration date.
• Your credit card billing address.
• Your daytime phone number.
• Your email address, if you have one.
• Please let us know if there is a specific program you want to support or if you are making an unrestricted gift.
