As a first step, our Geriatric Care Managers will visit you at home, getting to know you and learning about your preferences and lifestyle requirements. They’ll ask a wide range of questions, assessing your medical, personal, social and home care needs and then recommend a care management plan that’s right for you.

Once your personal care plan is in place, our Care Managers will arrange, coordinate and supervise your support services, finding compatible caregivers and making sure you have everything you require to live safely and fully supported in your own home and community.
