Our Geriatric Care Managers will work with you to design a customized care plan for your family members, coordinate and deliver the services you require, monitor your loved ones well being 24/7 and provide seamless transitions to new levels of care as needed. They will supervise all the services they provide, finding compatible caregivers and making sure your family members have everything they require to live independently.

What kind of qualifications do your Geriatric Care Manager’s have?
Senior Source’s Geriatric Care Managers are all highly trained members of the National Association of Professional Geriatric Care Managers. They hold primary licenses and advanced degrees in related fields such as social work, advanced practical nursing, medicine and law.

How are quality and costs controlled?

How does the process work?

  1. First, we’ll meet with you to get to know you and thoroughly understand your situation. We’ll assess you/your parent’s physical, mental, cognitive, emotional and environmental needs. Our interview will cover a range of questions including overall health, the safety of your home, your ability to participate in everyday activities, your nutritional status, your memory and cognitive functioning, your finances, your insurance coverage and more.
  2. Based on our findings, we’ll establish an affordable, customized care management plan. We’ll analyze your insurance plans, research benefits and entitlements and take immediate action to meet your current home health care needs. We’ll make sure you understand all the details of your care plan and let you know what you can expect from us. As your partner in your parents’ care, we’ll be available 24/7 to respond to your ongoing care requirements.
  3. As a client of Senior Source, you will have immediate access to the wide array of home care services available under one Selfhelp roof. We’ll provide you with skilled health care workers, aides who can help you perform the basic activities of daily life, escorts and transportation to your medical and other appointments and help with homemaking and housekeeping.
  4. Our Geriatric Care Managers are deeply connected to the community. They can direct you to a range of elder care resources from financial advisors, bookkeepers, and lawyers to social workers, senior centers and Alzheimer’s programs. They know the local elder care specialists, their reputations, which programs work and which programs to avoid.
  5. Monitoring and continuous support are part of the process. Geriatric Care Managers stay in contact with family members filling you in on your parents’ progress in a timely manner.
